Attaching a Form to a Shift

You can attach forms, checklists, surveys, tasks, and audits directly to shifts when building or editing your schedule. When you do this, the form is automatically assigned to the employee when their shift starts — no manual assignment needed each time.

Getting There

Go to Schedule from the left navigation → open or create a shift → look for the Forms dropdown at the top of the shift modal.

Attaching a Form When Creating or Editing a Shift

Inside the shift creation or edit modal, click the Forms dropdown at the top. You'll see these options: Audit, Checklist, Survey, Form, and Task. Select the type you want to attach, then search for and select the specific form from your library. You can attach multiple forms of different types to the same shift.

💡 Attaching a form to a shift is a one-time assignment tied to that specific shift. If you want a form to automatically attach every time a certain shift type is scheduled, set that up in the form's Settings → Assign to shifts instead.

What Happens When the Employee Clocks In

When an employee clocks into a shift that has a form attached, the form is automatically assigned to them. It will appear on their Home screen and in their Forms list under the Incomplete tab.

If Send reminder on start of shift is enabled in the form's settings, the employee will receive a notification when their shift starts.

Pre-Assigning Forms to Shifts via Form Settings

For recurring forms — like a daily opening checklist or a shift-end safety check — you can configure automatic shift assignment directly in the form's settings. Open the form in the form builder → click Settings → under Assign to shifts, select the shift type and choose which days of the week it should apply. Under Reminders you can enable shift-start reminders and follow-up reminders before end of shift.
