How to Create and Edit Forms

Forms let you standardize processes across your team — from training attendance and equipment sign-outs to daily checklists and employee surveys. You can build a form from scratch, start from a template, or let AI generate one for you.

Getting There

Go to Forms from the left navigation → click Create Form in the top right corner. To edit an existing form, hover over it in the list and click the pencil icon.

Step 1: Choose a Form Type

First, select the type of form you want to create:

  • Form — General data collection with flexible field types (signatures, uploads, text, etc.)

  • Checklist — Step-by-step items employees check off as they go

  • Survey — Collect structured feedback or responses from employees

You can also click Create form with AI at the bottom to have Teamsly generate the form for you based on a description.

Step 2: Create New or Start with a Template

Choose Create new to build from a blank canvas, or Start with template to browse Teamsly's built-in template library. Templates are a great starting point — you can select one, preview it on the right side, and then customize it fully in the editor.

Step 3: Build Your Questions

In the form builder, the left panel shows your question list and the right panel shows a live mobile preview. Click + Add Question to add a new field. Available field types are:

  • Text — Free-form text entry (toggle Multiline for longer responses)

  • Number — Numeric input

  • Date — Date picker

  • Yes/No — Two-button response with optional N/A. You can mark a correct answer for scoring.

  • Single Select — Choose one option from a list. Mark a correct answer for scoring.

  • Multiple Select — Choose one or more options. Mark correct answers for scoring.

  • Rating — Star or number rating scale

  • Checklist — Multiple items that can each be checked off

  • Task — An actionable to-do item

  • File Upload — Attach a document

  • Image Upload — Attach a photo

  • Signature — Employee draws their signature

  • Location — Captures GPS coordinates

  • Description — Instructional text block (not a question)

  • Header — Section divider to group questions visually

Drag questions up or down to reorder them. Toggle Required on any question to make it mandatory before submission.

📋 To enable scoring on a form, mark correct answers on Yes/No, Single Select, or Multiple Select questions. When an employee submits, their score is calculated automatically as a percentage of correctly answered questions.

Step 4: Configure Form Settings

Click Settings in the top right of the form builder to open the settings panel. Options include:

  • Form Name — Rename the form at any time

  • Role — Restrict which role can complete this form

  • Who can create → Allow employees to create — When checked, employees can self-initiate and submit it without a manager assigning it first

  • Assign to shifts — The form will automatically be assigned to employees when they clock into that shift type

  • Reminders → Send reminder on start of shift — Employee receives a notification at shift start

  • Reminders → Send reminder if user does not complete by [X] hours before end of shift — A follow-up reminder if still outstanding

  • Delete — Permanently delete the form

⚠️ Deleting a form from Settings removes it permanently. If you just want to stop using it temporarily, remove it from any shift assignments and stop assigning it manually instead.
