Completing a Form as an Employee

When a manager assigns you a form, checklist, survey, or task, you'll be notified and it will appear directly on your Home screen. You can also find and complete forms from the mobile app at any time.

Finding Your Forms on Mobile

The Forms screen has three tabs:

  • Incomplete — Forms that have been assigned to you but not yet submitted. Each form shows the name and the date it needs to be completed by.

  • Completed — Forms you have already submitted.

  • Available — Forms your manager has made available for you to start on your own, as many times as needed.

💡 Forms assigned to your shift will also appear on your Home screen as a to-do item. You'll receive a notification reminding you to complete them at shift start or before the shift ends, depending on how your manager configured it.

Completing a Form

Tap any form from the Incomplete tab (or from your Home screen) to open it. Work through each question from top to bottom. Depending on how the form was built, you may see text fields, Yes/No buttons, Single or Multiple Select options, checklist items, number or rating inputs, date pickers, location capture, image/file upload, or signature drawing. Questions marked with a red asterisk (*) are required.

📋 Some forms include scoring. Your manager may have marked certain answers as correct. Your score is calculated automatically once you submit — you don't need to do anything differently.

Submitting a Form

Once you've filled in all required fields, tap Submit at the bottom of the form. The form will move from the Incomplete tab to the Completed tab, and your manager will be able to see your submission.

Leaving a Comment

On any form, you can tap the speech bubble icon in the top right corner to open the Comments section. Use this to leave a note for your manager or ask a question about the form. Your manager can reply from the web.

Forms Available to Self-Start

If your manager has enabled Allow employees to create on a form, it will appear in the Available tab. You can open and submit these forms whenever you need to — they don't need to be assigned first.
