Creating a Form With AI

Teamsly can generate a complete form for you using AI. Instead of building questions one by one, you describe what the form should cover and the AI builds a draft you can review and edit. This works for Checklists, Surveys, and Forms.

Getting There

Go to Forms from the left navigation → click Create Form in the top right → on the form type selection screen, click Create form with AI at the bottom.

Step 1: Choose the Form Type

At the top of the Create with AI dialog, select the type of form you want to generate: Checklist, Survey, or Form.

Step 2: Describe the Form

In the text field, type a short description of what the form should cover. Be as specific as you can — the more context you give, the better the output. For example: "Workplace safety basics - key practices for staying safe on the job" or "Daily opening checklist for kitchen staff."

Step 3: Set the Number of Questions

Use the Number of Questions spinner to choose how many questions you want in the form. The default is 8.

Step 4: Choose Question Types (Optional)

You can check one or more Question Types to tell the AI which formats to use. If you leave all boxes unchecked, the AI will choose the most appropriate types on its own.

✨ Leaving the question types unchecked usually gives the best results — the AI picks types that fit the topic naturally.

Step 5: Generate and Review

Click Generate with AI. Teamsly will build a draft form based on your description. The form opens in the standard form builder where you can edit questions, change types, add or delete questions, reorder by dragging, mark questions as required, set correct answers for scoring, and adjust form Settings.

💡 The AI gives you a solid starting point, but always review the generated questions before assigning the form to your team. Adjust anything that doesn't match your specific process or terminology.
